Social workers assess the social care needs of patients and their carers and put packages of care in place for those going home; they also advise on and arrange nursing home placements.

They offer support both before and after bereavement to patients and families including children, working both individually and with groups. They train volunteers to specialise in bereavement support, working with them to deliver bereavement care.
